What is couples health insurance?
Couples health insurance is one policy that covers two adults. So, just like you might share a house with your partner, you could also share hospital and/or extras health insurance, too.
Different health funds may have their own definition of a couple when it comes to health insurance, but usually it covers married couples, de facto relationships or otherwise registered couples, no matter if it’s only been for a short time.
What could couples health insurance cover?
Health insurance for couples can be for hospital cover, extras cover or a combination of both (sometimes called ‘combined cover’). For instance, hospital insurance can cover you for in-hospital stays as a private patient for all kinds of treatments, like joint replacements or obstetrics. Meanwhile, extras cover is for services like major dental, physio, remedial massage or emergency ambulance cover.
With couples cover, both you and your partner will be covered for the same types of health care treatments.
How much is couples health insurance in Australia?
For iSelect customers in 2025, couples hospital cover cost on average $392 per month while couples extras cover cost $115 each month.1These figures reflect indicative couples hospital insurance policies, based on internal iSelect sales data and rounded where appropriate. Unlike other insurances, you can’t be penalised by your insurer if your health needs change, like having an additional loading added. All health insurance policies are community rated, even couples health cover. This means everyone pays the same standard price for a policy, no matter their age or health history – a little like how everyone gets the same Medicare rebate for a service.
How are premiums calculated for couples health insurance?
Health insurance premiums, including for couples cover, are influenced by the type and level of cover you choose, and the state you live in.
Different health funds may also price similar policies differently. For instance, while gold hospital cover tends to be the most expensive, some health funds might offer it at lower prices. Similarly, an extras policy with just general dental and optical tends to be on the cheaper side for all health funds, but comparing policies could help you find the one that offers you the most value for money.
Is couples health insurance cheaper than two singles policies?
Couples health insurance is usually about the same as two similar singles policies since health insurance premiums tend to factor in how many people are on the policy. So, couples health insurance can often be double the cost of a similar policy for one person.
However, depending on the level of cover you’re each after, and if you’re eligible for discounts or other perks, you may be able to shop around to find couples health insurance that is cheaper than two different options for yourselves. It could also work out as more convenient simply by virtue of only having one premium to pay. This could be particularly true if you opt for a combined hospital and extras policy.

Helpful tip

If you already have a singles health insurance policy, you may not need to go through the full application process to add your partner. Many insurers allow you to add new members to your existing private health cover – often with just a quick phone call. Some even offer an online portal where you can make the change in just minutes!
However, it’s worth comparing your options before making the switch, as your current insurer might not provide the best value or coverage for both of your health needs under a couples policy.

Should I get couples health insurance?
You should get couples health insurance if you want the convenience of a single policy and an increased threshold for the private health insurance rebate. However, you might prefer singles cover if you’re worried about reduced flexibility and the possibility of reduced medical privacy through shared access to your claims history.

When choosing couples health insurance, consider your individual health needs, set and stick to a budget, and compare options to find a policy you like.
For instance, consider each of your medical backgrounds, including hospital admissions and your families’ health histories, could help you figure out the level of healthcare coverage that gives you and your partner peace of mind. Ideally, you’ll be able to balance your desired level of cover with a regular premium you’re comfortable paying for.
To help narrow down the list, compare policies based on different features beyond cost, like how much you get back on a claim, annual limits and likely out-of-pocket costs; level of cover; additional membership perks and more.

What’s the best health insurance policy to get as a young couple?
There’s no one health insurance policy that’s best for young couples, with each couple’s needs being different. For example, basic or bronze hospital cover might be better for young couples that are healthy and unlikely to go to hospital for anything other than an unexpected accident. These policies give some cover while helping avoid the Medicare Levy Surcharge (MLS) for higher earners.
Alternatively, for couples planning to start a family, cover for pregnancy and birth-related services is found on gold policies or occasionally silver plus.
And don’t forget extras! These don’t have set tiers like hospital cover, so each policy might have different inclusions. If you’re both into sports or keeping a healthy set of pearly whites, you might be interested in an extras policy that covers things such as physiotherapy, acupuncture and dental check-ups.
The key is to shop around and compare policies to find one that suits your lifestyles, budget and plans. It’s not a one-size-fits-all deal, so take the time to figure out what’s important for you and your partner.
Do I need to be married to get couples insurance?
No, your eligibility for couples cover doesn’t depend on whether or not you’re married; most policies treat two adults living together as partners.
And if you’re in an LDR (long-distance relationship) – whether it’s for work, study, family commitments or even health treatment – you may still be able to hold a couples policy together, as long as you are both living in Australia.
If I have couples health insurance, do I still have to pay the MLS?
To avoid the MLS, you’ll need couples hospital cover with an excess of no more than $1,500. With this cover, even if you earn over the MLS income threshold, you’ll be exempt.
For the 2026–27 financial year, couples with a combined income of over $210,000 need to pay the MLS unless they have appropriate hospital cover. Your tier of hospital cover doesn’t matter, just your chosen excess.

How does Lifetime Health Cover (LHC) loading work with couples health insurance?
Couples LHC loading – the amount they’ll pay on top of their couples health insurance – is the average of their individual loadings. So, say you have a loading of 2% while your partner’s is 6% then the average is 4%.
You might have an LHC loading if you didn’t get hospital cover before the start of the next financial year (1 July) after your 31st birthday. Your LHC loading increases by 2% for each year beyond this that you don’t have hospital cover, all the way to a maximum of 70%. That’s why couples young and old alike may want to get covered sooner rather than later.
If you’re only after extras cover, this kind of cover won’t stop you from getting an LHC loading. However, LHC loadings aren’t applied to extras policies either, so you won’t need to worry about your extras premium going up because of this.
Can a divorced couple share a policy?
Divorced couples can share a policy, but it will ultimately come down to what their insurer decides – as each has its own definition of a couple for health insurance, with some including divorced couples and others not. You can read the policy documents to check or speak with the insurer. There may also be tax implications.
Can I add dependents or children to couples health insurance?
You can’t add dependents to a couples health insurance policy without changing it to a family policy. Your insurer might have a family version of your existing policy you can move to. Otherwise, you can look at different family health insurance policies. This can be a good idea since your health needs as a family might be different to what they were when it was just the two of you.
Once you’re on a family policy, you can then continue to add dependents and children to your cover if you’re looking to expand your family further.
What if one person has a pre-existing condition on couples health insurance policy?
Health insurers can’t legally refuse to insure you or your partner if one or both of you have a pre-existing condition. However, pre-existing conditions usually mean a waiting period before you start claiming (unless you’ve already served that waiting period with another insurer).
For hospital cover, you won’t have to serve more than a 12-month waiting period before you can claim for hospital treatment. For extras cover, your insurer can set any length of waiting period and pre-existing conditions aren’t a factor.
If you’re taking out a new policy with your partner and one of you has a pre-existing condition, it’s usually only that person who would have to wait out the pre-existing waiting period before they can claim, rather than both of you. It really all depends on the policy, so don’t be afraid to speak with your insurer or go through your policy documents.
